Scope of Services
- Membership services are limited to conditions appropriate for, and within the scope of telehealth
- Certain conditions may require in-person evaluation
- Membership does not include imaging or lab fees, pharmacy costs, or services provided by third parties
- Membership does not guarantee treatment, prescriptions, or referrals; all care is provided at the clinician's discretion
Payment Terms
- The membership fee is $180 per year, billed annually
- Payment is due in full at the time of enrollment
- Membership fees are non-refundable except as required by law
- Membership begins on the date of payment and is valid for 12 months
- Membership automatically expires at the end of the 12-month period unless renewed
Renewal Instructions
Renewals are billed annually. Members will receive a reminder email approximately 30 days prior to their membership expiring.
